National Exercise Referral Scheme (NERS)
Provides support and high quality and varied exercise opportunities for individuals, aged 16 and over, whose health would benefit from a more active lifestyle. NERS is designed for people who are not physically active and have a medical condition.
It provides opportunities to exercise that are fun, rewarding and that can be incorporated into everyday life including gym sessions, gentle exercise classes, swimming,yoga, walking, COPD classes, Mature movers and aqua aerobics. To access the scheme, speak to your GP or other health professional who will be able to refer you. You will be contacted to arrange a consultation, find out a bit moreabout your health history and help select a programme of exercise suitable for you

Road to Wellbeing
Many of us experience difficult times in our lives which can leave us feeling stressed, anxious or depressed. The Road to Wellbeing programme teaches people how to manage their stress and improve mental wellbeing through its open access courses: "Stress Control" and ACTivate Your Life".
There are also a range of self-help resources available on the website, including downloadable self-help guides and links to online Cognitive Behavioural Therapy (CBT) and Mindfulness apps.

Communities for work plus employment support
Communities for work plus offers individualised 1:1 support from community employment mentors, access to work clubs, volunteering opportunities, free vocational training, CV writing, interview and job application support. Wider provision to support with financial issues and health and wellbeing is also available. Digital support is also offered to support with maintaining Universal Credit claims online.
Mentor work at the pace of each participant, in line with their needs and any conditions they may be experiencing, with access to suitable placements that can help with social isolation and trailing work activity with your condition in mind.

Disability Advice Project
Disability Advice Project provides FREE help with the following:
- Advice on benefits
- Completing benefit claim forms
- Gathering evidence to support claims
- Support with Mandatory Reconsiderations
- Representations at appeal
- Advice on aids and equipment
- Advice on accessible holidays
- Advice on community care assessments
- Resilience training
- Assertiveness training
They are open Monday to Thursday 10am to 4pm.

Disability Sport Wales
Disability Sport Wales helps to identify appropriate sport and health and wellbeing opportunities for individuals, identify gaps in provisions and either support the expansion of existing provisions or help create new clubs and groups when appropriate throughthe identification of funding.

Torfaen and Blaenau Gwent Mind
They can provide;
- A daily drop in to their mental health and housing surgery for advice and guidance on how to access floating support and their STEP service for support around your mental health, wellbeing and tenancy.

Education programme for patients (EPP)
EPP Cymru provides a number of free self management courses for people living with any long-term health condition or in a caring role. These courses provide an opportunity for people to learn new coping skills, which can help improve the quality of daily life.

Fibromyalgia support groups
Fibromyalgia support groups offer an opportunity to meet others with the same condition, share experiences and find out more about the support available. They are held monthly on a Tuesday 1-3pm at The Settlement, Trosnant Street, Pontypool NP4 8ET.

Building Resilient Communities
The new Building Resilient Communities Programme is aimed at supporting the most vulnerable people or communities within Torfaen. The programme facilitates early intervention and prevention. Providing support to build resilience of families, children and adults.
The Resilient Physical Wellbeing Resilience Officer can help with:
- One to one and small groups
- Confidence building
- Low intensity walking and gym work
