Make sure your local pharmacy knows the details of the medicines should be taken.
Every year thousands of people go back into hospital because of problems with their medications. DON'T LET IT BE YOU!!!
What's this about?
- This service will help make sure that you, your GP and your local pharmacy have the same information about your medications after you get home. This will help to make sure you continue to take your medicines as intended.
- Please discuss the service with your hospital pharmacist. You will need to provide details of the local pharmacy from which you usually get your prescriptions.
- You may help to sign a consent form to give permission for information about your condition, treatment plan and medicines to be given to your local pharmacy.
The pharmacy will:
- Check to ensure that you have all the medicines you need and that you have stopped taking and no longer need
- Explain what your medicines do and how to get the best results.
- Work with your GP to make sure your new prescription is accurate
- Arrange a follow up consultation with you to make sure your medication are continuing to work correctly.
The NHS Discharge Medication Service is only available from your local pharmacy.
